Checkmating Infant Mortality In Nigeria3 by Soji Omole
The infant mortality rate is 75 deaths per 1,000 live, child mortality (age 12 months to 4 years) is 88 deaths per 1,000 live births, while the overall under-five mortality rate is 157 deaths per 1,000 live births. Although there has been a decline in the under-five mortality rate from 201 in the 2003 NDHS to 157 deaths per 1,000 live births in the 2008 NDHS, this 20% decline indicates that the annual rate of progress made to date is insufficient to realize the health related MDGs. Easily preventable and/or treatable infectious diseases account for most of these being attributable to malaria (24%), pneumonia (20%), diarrhoea (16%) and measles (6%) (Figure 1). It is evident that if the lives of children are to be saved in the country, there will be a need to scale up a core package of proven evidence based interventions.
Out of Nigeria's population of 148 million, women of childbearing age constitute 31 million, out of which the country loses 145 women every day. Annually, an estimated 52,900 maternal deaths occur in Nigeria, and a woman's chance of dying from pregnancy and childbirth in Nigeria is 1 in 13 (FMOH, 2003). The major causes of maternal death include haemorrhage (23%), infections (17%), toxaemia/eclampsia (11%), obstructed labour (11%), unsafe abortion (11%), malaria (11%) and anaemia (11%). Coverage of key Maternal and Child interventions is still low with focus on single disease entities as opposed to the delivery of a comprehensive package of evidence based core interventions. Prioritization of programs such as Polio Eradication, malaria control and measles elimination are mostly based on donor preferences and are being scaled up in campaign modes that are often more expensive vertical approaches. As a result, the routine health system remains weak. The revamping of the Primary Health Care System should be a priority as the current three-tier national healthcare system is in a crisis state, struggling to meet even the bare minimum of any national healthcare.

It is against this backdrop that Nigeria initiated the implementation of the National Integrated Maternal, Newborn and Child Health (IMNCH) Strategy to fast-track high-impact intervention packages that include among several others, immunization, nutritional supplementation, midwives service scheme, provision of anti-shock garments, insecticide-treated mosquito nets distribution and prevention of mother-to-child transmission of HIV.
The strategy, rolled out in three phases each lasting three years, has been designed along the continuum of care model to strengthen Nigeria's health system at all levels - federal, state and local. The strategy is on the way to bring about improvements which will accelerate the country towards meeting Millennium Development Goals four and five.
One of the programmes aimed at confronting this challenge eyeball to eyeball, the Maternal and newborn Child Health Week (MNCHW) refers to a week-long event of accelerated actions, flagged off in all the thirty six states of Nigeria, including the Federal Capital Territory ((FCT). This is to promote the health and wellbeing of mothers and children in Nigeria. During the MNCHW, each Local Government Areas will deliver high impact interventions both oral, intramuscular, intradermal and intramuscular vaccines through existing routine and/or expanded outreach services. The activities include, among others, vitamin A supplementation, routine and catch up immunization for children and tetanus toxoid for pregnant mothers, de-worming, and screening of malnourished children and promotion of selected key household care practices such as breastfeeding, complementary feeding and hygiene.
